Urgent Telehealth Information for Families Receiving Early Intervention Services
If you are currently receiving EI services through an SLP Communication Foundations provider and would like to receive services via telehealth during the state of emergency please download and complete the form attached. NYS requires all families who wish to participate complete and sign the consent form before teleservices can begin. We ask that you complete your child’s name, DOB, and address along with the bottom section of the form. Upon completion of the form please return to twarnstadt@langfun.com or you can mail it to 1032 Main St. Fishkill NY 12524. We have also attached the “Family Rights in Early Intervention” for your reference
